Office Policies

Prescriptions:
Refill request for all controlled medications that are not picked up within the seven days allowed and must be rewritten there will be a $5.00 fee for each prescription. Please remember to pick these up in a timely manner, as they are valid for 7 days only.  The person for whom the controlled substance prescription is written will be required to show ID and sign for the prescription.  If the patient is a minor, the parent/guardian will be required to sign for the prescription.

Our physicians are using (or will be using soon) an electronic ordering system for medications that are not controlled substances.  The physician will submit your prescription online and send to the pharmacy of your choice. 

Photocopies:
Medical records requests for patient's purpose only will be $25.00 for the first 20 pages and $.15 for each additional page.  If another physician’s office requests chart information, there is no cost to the patient.  The forms will be sent directly to the requesting physician’s office.

Forms Completion:
School, camp, sports, employment physicals, disability forms, etc. completed will be $25.00 per form. Payment will be required before paperwork is picked up.

Sample Medications:
Please pick these up within 48 hours of being notified that the medication is ready; as they will be returned to stock after this time and may not be available after the 48 hours.

Refills:
All refills will be handled within
48 business hours. Please contact your pharmacy to request your routine medications, as they will contact us for authorization to refill the medication. Allow yourself an ample supply so that you do not run out at the last minute. To help with the tremendous amount of phone calls that come in please wait 48 hours before contacting the office about your prescriptions.  If it has been less than 48 hours since the first contact about medication, the medical assistant or nurses will not be able to return your call concerning the medication refill.

Weekend and Holiday refills:
Make sure all medication refills are handled before Thursday at 4:00pm and one day before a holiday; otherwise you will need to wait until the next Monday or regular business day. No refills will be called in to a pharmacy by the physician on call over the weekend or holiday.

Lab Results:
If your lab results are normal and the physician has no instructions for you, a copy will be mailed to you with 2 weeks.  The medical assistant or nurse will attempt to contact a patient twice if there are instructions that the physician is giving related to the labs.  If the patient does not return calls within two weeks, a copy of the lab and instructions will be mailed to the patient.  The information can be left on voicemail if the patient has filled out and signed a release giving us permission to leave information at a particular phone number. 

 Lab Draws:
On occasion, you may need labs drawn due to numerous reasons.  The medical assistant or nurse will fill out the requisition that you will need to take to the lab for processing.  In some cases, you may be given several lab slips for different days/months.  Please be aware of which one you are turning in to the lab for processing.  If you leave a message requesting labs, please allow 48 hours for processing before arriving to pick up the lab slips. 

Leaving Phone Messages with Information:
When leaving a message for your physician, please give as much detail as possible, as this will help in his/her decision process.  All information left is confidential and protected under HIPAA regulations.
